Mass Families Organizing for Change (MFOFC) wants to know if you have provided substantial consultation to one or more of the state agencies named in the Chapter 171, Individual and Family Support Law.

This Massachusetts law requires each of the seven named agencies to develop an annual Individual and Family Support agency plan with specific objectives based on the input they receive from their consumers (the individual with disability and their families) about the flexible supports each state agency provides.
